  11.   Functional limit switches
  12.   Alignment of deck support
  13.   Broken castors / axles
  14.   Wall / floor attachment secure
  15.   Broken safety end rails / handi-cap rails
  16.   All moving parts cleaned and lubricated

Of the above items, numbers 1,2,3,4,6,7,8,10,14 & 15 are all safety problems and should be repaired immediately.
